Choose a long term condition that you have studied in class or seen on a placement and write a case study on the condition highlighting the following:

Nursing considerations
Medication side effects
Maintenance and management of the condition

You can choose from the long termconditions (including the acute phase) that have been discussed in class or you can write about a patient with a long term condition (including the acute phase) that you have seen whilst on placement. Please refer to the examples discussed in the workshops/seminars to understand the details required for that particular patient you are planning to write about. This assignment should demonstrate your understanding of the condition and what you have learnt during the term
